The order, suspect and recheck attributes of the concept_id element are optional. If left blank, suspect and recheck are set to "0".
If order is left blank, the order in which the concepts ids appear in the document is then used.It is essential that, for a given snomed_group, either ALL or NONE of the concept_id elements have order attributes. The results are
undefined if some orders are defined and others are not.
